Guidance Notes for Completing Questionnaire
The questionnaire is split into 4 sections.
- Section A: Main contact details
- Section B: Your organisation and its services
- Section C: Organisation operation
- Section D: Your consent
Section A: Main contact details
This is for basic general contact information. Please take care to ensure
that details are clear and accurate.
The purpose of the logo is to give your entry in the directory a greater identity.
Section B: Your organisation and its services
The purpose of this section is to describe your organisation to the users, and to help us best place your entry in the resource guide.
- Summary, a brief statement, or a mission; a brief description of your organisation.
- Which categories best describe your organisation? Which should it fall under? Please put a number "1" against the category that best describes the work of your organisation. This is the area of the Resource Guide that your entry will appear in. If you feel that your work is appropriate to other categories, please indicate up to four other categories and we will include a cross reference in these categories directing people to your main entry.
- Status of your organisation: Voluntary or Statutory?
- Who is your main target group? Who would benefit from your services? Please ensure that you do not give more than 5 answers, determine the 5 most common client types and select them as your choice. The reason we ask you not to choose more than 5 answers is because if you select too many boxes, it will end up giving un-useful information to the users.
- Which services does your organisation provide? Again, do not give more than 5 answers. The selections you make here will be used for a cross-reference section to help users identify the help they require.
Section C: Organisation operation
This section allows you to give more details about how your organisation operates.
- Does your organisation charge its users a fee to use the services you provide?
- How do people make contact with you? Who can refer people to your group?
- Referral and Self-Referral paths: how can the users access your services?
- Further comments? Perhaps there are certain areas of your organisation the questionnaire did not cover and you may wish to add in?
If need be, you can use another piece of paper to complete.
Section D: Your consent
We require your permission to publish the information you have submitted to us for the Resource Guide. The guide will be published in hard copy format, as well as for the web. Please tick the boxes if you agree to MVA publishing your details, and ensure this section is signed by someone authorised to give such permission in your group.
